Directions for Completing the General Petition Form
NOTE: The General Petition form is only for extenuating circumstances, undue hardship, or other conditions beyond your control. You are requesting an exception to a standing University policy that applies to all students in similar programs.
1. Fill out the form completely.
2. Provide a detailed rationale to support your request. Rationale that will not be approved include “I forgot”, “I didn’t know”, “I was busy”, etc. Attach any related documents that may help in your case.
3. Your advisor will be notified of your petition and will need to forward their approval to the Registrar's Office. (Please note that the advisor does have the right to deny your petition.)
4. Your petition will be reviewed by the Petition Committee at their weekly meeting.
5. You will be notified of the decision for your petition within 5 business days of the scheduled Petition Committee meeting.
